I know a dealer can know if I have made modifications to the ECU even after uninstalling the access port. However, should I uninstall the access port if I am taking the car for an oil change or the soon to come 30k maintenance? So it not so obvious, or it is ok to leave it install?

I'd take the device out so that it doesn't get stolen, but like you said, no point in uninstalling. They'll know either way if they want to look into it.
