It's a permission on the table in MySQL. When the connect function in the include file gets called, it fails authorization. The include file error line should be the function call, not where to find the failing table. Looking the in the index file for that directory to find out which php file is calling the include, it should tell you the table that it's trying to access.
Someone with server level access will need to address this one.
