Here is my process in Photoshop...
First make sure you have your inner border color as your top color and your outer border color as your bottom color
CTRL-A to select all
Edit, Stroke...make sure you select Inner...I use 1 px for this.
Then, Image, Canvas Size...and then increase the canvas size to the same amount of pixels on both width and height. I normally have my pictures shruken down to 770x512, so I then increase my canvas size to 800x542 to give a 15-pixel border on each side.
End result:

Yep, I do it through canvas size as well. On an unedited 8mp file I usually use a 1inch by 1inch increase "relative" and select the bkg color as black. Just remember if you do a large stroke you are covering up the outside of the picture, when it is just a few pixels it is no big deal but I wouldn't do it with a normal sized border.
